(b) personal information about a healthcare practitioner’s patient (“End User”) provided by that healthcare practitioner for the purpose requesting that The Herb Booth supply and mail a product to the End User on behalf of the healthcare practitioner, as prescribed by that healthcare practitioner (“End User Personal Information”). This type of personal information collected by The Herb Booth may include, but not be limited to, the name, date of birth, and address details of the End User.
Use and Disclosure of Information
End User Personal Information collected by The Herb Booth will only be used for the purpose of preparing and posting products to an End User at the request of a healthcare practitioner, unless an End User consents to its use for other purposes or the law permits its use for other purposes.
Practitioner Personal Information collected by The Herb Booth will only be used:
(a) to provide the products and services requested by that healthcare practitioner from The Herb Booth;
(b) to provide that healthcare practitioner with information relating to The Herb Booth’s products and services;
unless a healthcare practitioner consents to its use for other purposes or the law permits its use for other purposes.
The Herb Booth may disclose End User Personal Information and Practitioner Personal Information to, or authorise collection of End User Personal Information and Practitioner Personal Information by, third parties who assist The Herb Booth in carrying out its services (“Third Party Service Provider”). Without limiting the foregoing, The Herb Booth may use the server of a Third Party Service Provider to collect and store personal information provided through the Site.
Security
Information submitted through The Herb Booth Site is via a secure web browser, which encrypts the information using Secure Sockets Layer (SSL) technology.
